WHAT WE DO
More Than Skilled Nursing, Less Than A Hospital
Pinnacle Medical Care Homes are Congregate Living Health Facilities (CLHF) and are based out of residential homes. We provide Inpatient Medical Care in a home setting. The care provided is generally less intense than what is provided in general acute care hospitals, but more intense than what is provided in a Skilled Nursing Facility (SNF)
BASIC SERVICES PROVIDED
Our Medical Care Homes, are residential homes with a capacity of 6 persons. We provide inpatient care, including the following basic services
Medical Supervision
24 - hour skilled nursing and supportive care
Provide skilled nursing on recurring, intermittent, extended or continuous basis for patients in need
Provide all aspects of daily living while under medical care such as: medication management and administration, assistance with daily living, cooking and cleaning, and activities

LEVEL 1 - SKILLED NURSING CARE
LEVEL 2 - REHABILITATION SKILLED NURSING CARE
LEVEL 3 - EXTENSIVE SKILLED NURSING CARE
LEVEL 4(A) - TRACHEOSTOMY CARE
LEVEL 5 (B) - VENTILATOR CARE WITH OR WITHOUT REHABILITATION
